Patient Passport Program and Resources

What is the Patient Passport?
The Patient Passport is similar to a travel passport but it is meant for people to use as they travel through the healthcare system.  The passport will contain copies of Advance Directives, Personal Preferences, Communication Contact Information, CPR/MOST documents, Medication Lists and a Patient Passport document of essential healthcare information.  The resource is portable, customizable and can be used as a resource in an electronic health record. Why is the Patient Passport a Good Idea?
  • Poor communication - can result in healthcare errors when essential health information is not fully conveyed
  • Medication issues - older adults are often prescribed potentially inappropriate medications
  • Advance Directives - a central place to keep all wishes and preferences for treatment
  • Dementia - essential communication tips to each patient can be recorded

With help from the Health District of Northern Larimer County's Larimer Advance Care Planning Team, the Patient Passport is in it's first user pilot program and will be distributed to 400 individuals by the end of 2019.
